What documents do you need
List of medical documents
- Physical exam form signed, stamped and filled out by the doctor
- Evidence of PPD to be updated yearly
OR
- Evidence chest x-ray (if the PPD was positive)
- Evidence of rubella titer or rubella vaccine separate report
- Evidence of measles (rubeola) titer or measles vaccine (if born on after 01/01/1957 separate report)
- Drug Screen (separate report) to be update yearly
